2024-2025 LAVBC SEASON

USAV Club Commitment Form

Please sign this form if you are committing to play for LAVBC during the 2024-2025 season. Forms signed before 8:00AM on 7/28 will not be accepted.

COMMITMENTINFORMATION:

  1. Each player and a parent/guardian will sign a Letter of Commitment to secure a place within a given club.

  2. The Letter of Commitment may not be signed until 8:00am on September 29, 2024, for players aged 7-14.

  3. After 8:00am on July 29, 2024, a player and parent/guardian may sign and submit a Letter of Commitment and Deposit to one and only one club. Players may not sign more than one Commitment Letter per season.

  4. The player will then be committed to this club for the entire 2024/2025 season.
